It doesn't look like the seeder attachment for the Power-Ho's or the Gardener tractors. ....It may possibly be for the Handi-Ho (one-wheel tractor) since it has the large rear wheel.

It does appear that it has been modified.

That is what I'm thinking

Yes it been modified, but the basic unit was built to fit the 15F tractors. What missing is the correct hitch, that would have looked like the front part of a cultivator attachment. These seeder units were built for several differnt companies, and most were only differnt in the hitching area. They were built by Danville Manf. in Danville, ILL during the 50's and on up into the 70's. Most of the time you see them on a David Bradley walk behinds, and later as a 3pt unit on the Suburbans. The tag is probally correct for a Bolens. I have one for a Bolens walker here, but it in pretty poor shape.
